to set up an action plan and track progress.
Copy the worksheet as needed.
- Identify
program areas where elements are missing
or defective.
- Determine
where you want to be.
- Identify
objectives, which will help you reach
your goal.
- Divide
objectives into specific activities.
- Assign
responsibility to one individual who
will be held accountable for the activity.
- Assign
target dates to each activity for
evaluation purposes.
